De Novo created a new regulatory category
TytoCare announced in April 2026 that the FDA had granted De Novo classification to the first model in its ENT Suite. De Novo covers novel low- to moderate-risk devices without a suitable predecessor, and the decision applies to a specific intended use rather than general AI diagnosis of every ear condition.
The model flags a bulging eardrum in compatible-otoscope video. Doctors still consider pain, fever, duration, age and other findings. The output supports a provider and informs a patient; it is not an automatic antibiotic prescription.
Capturing a good video may be the hardest part
Otoscopy needs the right angle, light and view. A moving child, wax or narrow canal can obstruct the eardrum, so a home device must guide the user and reject inadequate recordings. Ergonomics, instructions and image-quality checks matter as much as classification.
TytoCare cites about 1.6 million ear images and records, but composition and labels matter more than size alone. Validation should be separate from training and report sensitivity, specificity and especially the rate of unusable home recordings across ages, skin tones, devices and settings.
AI can add a physical finding to a remote visit
An ordinary video call cannot listen to lungs or look properly into an ear. TytoCare's home kit brings these measurements into a remote consultation, while AI can standardise recording and flag a sign for attention.
The benefit is greatest with assured follow-up. Providers must define who receives the result, response time and when an in-person examination is needed. Serious symptoms should never be delayed by a home test; the product is a triage and care service as well as hardware and an algorithm.
More accurate antibiotic use may be the goal
Ear infections are a common reason for paediatric visits, and treatment is difficult without a good view. Better recording may reduce unnecessary antibiotics or delayed care, but studies must measure prescribing, repeat visits, complications and family experience rather than image accuracy alone.
A negative result must not create false reassurance and a positive result does not prove an infection needing antibiotics. Patient-facing language should communicate uncertainty and support shared decisions rather than treatment pressure.
A home device needs operational discipline
The device must be clean, charged, connected and assigned to the correct patient, with tips replaced and disinfected. Software must protect health data, update the model and preserve audits. Families unable to make a recording need support and an alternative care path.
Economics depend on utilisation and must include lost devices, support, consultations and cases still needing an office visit. Company-wide user numbers do not prove return in the differently organised Czech primary-care system.
European use requires local assessment
US classification is meaningful but Europe has its own rules. Customers must verify conformity marking, local-language instructions, data handling and clinical responsibility. Czech record integration and clinician availability matter more than the ability to buy the device. A pilot should track usable recordings, consultation time and real effects on visits and treatment.
The product joins Israeli experience in medical devices, telemedicine and machine learning around one precise task. It does not replace a paediatrician, but could fix an important weakness of video consultations if it reliably improves the remote view and preserves safe follow-up.
